Violation of Code: [511-6-1.07(3)(a)] Upon entrance; observed no hand soap or paper towels at employee or restroom hand wash sink. Each handwashing sink or group of two adjacent handwashing sinks shall be provided with a supply of hand cleaning liquid, powder, or bar soap. COS: PIC provided hand soap and paper towels for both sinks. Corrected On-Site. New Violation. 8-2B Violation of Code: [511-6-1.07(6)(g)] Observed sanitizer spray bottle with 500+ ppm of Quaternary ammonium. Chemical sanitizers, including chemical sanitizing solutions generated on-site, and other chemical antimicrobials applied to food-contact surfaces shall: Meet requirements specified in 40 CFR 180.940 Tolerance exemptions for active and inert ingredients for use in antimicrobial formulations (Food-contact surface sanitizing solutions. (QAC sanitizer must be 200-400 ppm) COS: Inspector intervened and educated employees on proper concentrations of sanitizer and remade to a concentration in compliance. Corrected On-Site. Repeat Violation. 10D Violation of Code: [511-6-1.04(8)(b)] Observed buttermilk, and half & half to be stored in a reach in cooler with expiration dates of 1/23/23 and 1/4/23. Milk, and time/temperature control safety foods that are labeled as “keep refrigerated” and that are for sale or service to the consumer or used as an ingredient in other foods shall be immediately discarded and shall not be sold, served, or used after the manufacturer’s expiration date or the sell-by date. COS: PIC discarded out of date dairy items. Corrected On-Site. New Violation. 14B Violation of Code: [511-6-1.05(10)(e)1,2,4] Observed recently washed, rinse and sanitized dishes stored wet and failing to be placed in a self draining position. cleaned equipment and utensils, laundered linens, and single-service and single-use articles shall be stored: (i) In a clean, dry location;(ii) Where they are not exposed to splash, dust, or other contamination; and (iii) At least 6 inches (15 centimeters) above the floor.2. Clean equipment and utensils shall be stored as specified under paragraph 1 of this subsection and shall be stored:(i) In a self-draining position that allows air drying; and(ii) Covered or inverted. 4. Items that are kept in closed packages may be stored less than 6 inches (15 centimeters) above the floor on dollies, pallets, racks, and skids. COS: Employee re-wash rinsed and sanitized items and placed in inverted positioning. Corrected On-Site. New Violation. 15B Violation of Code: [511-6-1.05(3)(h),(i)] Facility could not provide test strips for testing the concentrations of chlorine and quaternary ammonium based sanitizers. A test kit or other device that accurately measures the concentration in mg/L of sanitizing solutions shall be provided. Facility must provide test strips for facility by COB. New Violation.
