Violation of Code: [511-6-1.03(3)(c)] Observed no posting of CFSM certification in facility for public viewing. The original CFSM certificate shall be posted in public view in each food service establishment. An additional copy shall be retained on file at the food service establishment at all times, and shall be made available for inspection by the Health Authority. CFSM identified and will post certification at COB. New Violation. Correct By: 10/11/2022 6-2 Violation of Code: [511-6-1.04(6)(h)] Observed TCS items with discard prep dates of 09/26 and 10/3 holding in reach in coolers. A food that requires datemarking shall be discarded if it:(i) Exceeds 7 days, not including the time that the product is frozen;(ii) Is in a container or package that does not bear a date or day;(iii) Is appropriately marked with a date or day that exceeds 7 days. COS: Items were voluntarily discarded during inspection. Corrected On-Site. New Violation. 17C Violation of Code: [511-6-1.07(5)(a),(b)] Observed pipe leak at front food service employee hand wash sink. Facility to get serviced within (5) days time. All physical facilities shall be maintained in good repair. New Violation. Correct By: 10/14/2022
