Violation of Code: [511-6-1.04(3)(e)] Observed soda bag had ruptured and placed in a plastic bin on floor. 511-6-1.04(3)(e) - Package Integrity (Pf) (e) Package Integrity. Food packages shall be in good condition and protect the integrity of the contents so that the food is not exposed to adulteration or potential contaminants. Pf COS-Soda bag was discarded during the inspection. Corrected On-Site. New Violation. 4-2B Observed accumulated mold debris inside the ice making machine in the back kitchen area. Observed accumulation of mold debris around the ice cream making machine. Observed sanitization cloths stored in 0ppm Cl Observed the 3 compartment dish sink with 10ppm Cl. The last of the bleach solution had been used, during the inspection manager sent employee to store to purchase bleach and corrected the sanitization solution to 50-100ppm Cl All food contact surfaces are to be clean to sight and touch. Note: Manager will clean the ice machine at the end of the day. New Violation Corrected On-Site. New Violation. 15A Violation of Code: [511-6-1.05(1)(a)] Observed 3 compartment sink using plastic wrapping for drain stopper in sinks, provide commercial drain stoppers. Observed the interior of ice machine door had been painted with silver paint and is flaking. Replace ice machine door. 511-6-1.05(1)(a) - Materials, General Requirements (P, C) (1) Materials. (a) General Requirements. Utensils and food-contact surfaces of equipment shall be made of materials that do not allow the migration of deleterious substances or impart colors, odors, or tastes to food and under normal use conditions, and shall be: P safe P, durable, corrosion-resistant, and nonabsorbent. In addition, materials shall be sufficient in weight and thickness to withstand repeated warewashing and shall be finished to have a smooth, easily cleanable surface. Materials shall be resistant to pitting, chipping, crazing, scratching, scoring, distortion and decomposition. New Violation. 16B Violation of Code: [511-6-1.06(2)(e)] Observed Vacuum breakers missing at hose bibs at the 4 compartment sink and the pre-rinse area before the dishwasher. 511-6-1.06(2)(e) - Backflow Prevention Device, Design Standard (P) (e) Backflow Prevention Device, Design Standard. A backflow or backsiphonage prevention device installed on a water supply system shall meet American Society of Sanitary Engineering (A.S.S.E.) standards for construction, installation, maintenance, inspection, and testing for that specific application and type of device. P New Violation. Violation of Code: [511-6-1.07(3)(f)] Observed the threshold and door gasket of the walk-in freezer in disrepair. Observed missing light bulbs in the vegetable prep walk-in cooler. 2. At least 20 foot candles (215 lux): (i) At a surface where food is provided for consumer self-service such as buffets and salad bars or where fresh produce or packaged foods are sold or offered for consumption; (ii) Inside equipment such as reach-in and under-counter refrigerators; New Violation. 18 Violation of Code: [511-6-1.07(2)(k)] Observed fly paper in use in the dry storage and food prep area. 511-6-1.07(2)(k) - Insect Control Devices (C) (k) Insect Control Devices, Design and Installation. 1. Insect control devices that are used to electrocute or stun flying insects shall be designed to retain the insect within the device. 2. Insect control devices shall be installed so that: (i) The devices are not located over a food preparation area; and (ii) Dead insects and insect fragments are prevented from contact with exposed food; clean equipment, utensils, and linens; and unwrapped single-service and single-use articles.
