Violation of Code: [511-6-1.03(2)(a)-(l)(n)(o)] Observed a child that is not employed at establishment run in and out of main kitchen area on (2) occasions and handled raw meat. Persons unnecessary to the food service establishment operation are not allowed in the food preparation, food storage, or warewashing areas, except that brief visits and tours may be authorized by the person in charge if steps are taken to ensure that exposed food; clean equipment, utensils, and linens; and unwrapped single-service and single-use articles are protected from contamination. COS: PIC was educated on authorized personnel access and child was dismissed from kitchen area. Corrected On-Site. New Violation. 1-2A Violation of Code: [511-6-1.03(2)(a)-(l)(n)(o)] PIC failed in his duties to effectively monitor: cold holding, authorized personnel access; knowledge & maintaining Employee health agreement contracts; hand sink operation and maintenance and sanitizing concentration and cleaning of food contact surfaces. Retraining of the PIC is recommended to enable him to efficiently fulfill his duties to educate and train food staff and practice effective Active Managerial Control of foodborne illness risk factors. PIC was educated on all these items during the inspection. Corrected On-Site. New Violation. 2-2A Violation of Code: [511-6-1.03(2)(m)] PIC could not furnish any verifiable documentation that employees know when to report or are knowledgeable of health activities and diseases. Food employees and conditional employees are informed in a verifiable manner of their responsibility to report in accordance with the Chapter, to the person in charge, information about their health and activities as they relate to diseases that are transmissible through food. COS: Inspector provided DPH employee heath agreements to PIC and encouraged training to every employee on potential Food-Bourne illness exposure while working in a food service establishment. Corrected On-Site. New Violation. 2-2D Violation of Code: [511-6-1.06(2)(o)] Observed a metal part used for tortillas stored in employee hand wash sink. A hand sink shall be maintained so that it is accessible at all times for employee use. COS: An employee removed the metal item immediately. Corrected On-Site. New Violation. 4-2B Violation of Code: [511-6-1.05(7)(a)1] Observed ice machine with an accumulation of mold and grime along the interior surfaces and chute. Equipment food-contact surfaces and utensils shall be clean to sight and touch. PIC to properly drain and clean ice machine at COB. New Violation. Correct By: 08/19/2022 4-2B Violation of Code: [511-6-1.05(6)(n)] Observed 3 Compartment sink sanitizer to register 0 ppm of Chlorine. Chlorine sanitizer must have a concentration of 50-100 ppm. COS: PIC remade sanitizer to 50 ppm. New Violation. 6-1A Violation of Code: [511-6-1.04(6)(f)] Observed multiple TCS items cold holding in a prep top cooler to have temperatures ranging between 55F-60F. (see log.) Time/temperature control for safety food shall be maintained at 41°F (5°C) or below. COS: PIC discarded items and moved eligible items to colder holding unit. Corrected On-Site. New Violation. 11A Violation of Code: [511-6-1.04(6)(e)] Observed pot of meat soup sitting at 97F on a counter with no cooling method to ensure proper cooling. PIC explained food had been sitting there for roughly an hour and was cooling down. Cooling shall be accomplished in accordance with the time and temperature criteria specified under DPH Rule 511-6-1-.04(6)(d) by using one or more of the following methods depending on the type of food being cooled: (i) Placing the food in shallow pans; (ii) Separating the food into smaller or thinner portions; (iii) Using rapid cooling equipment.(iv) Stirring the food in a container placed in an ice water bath; (v) Using containers that facilitate heat transfer; (vi) Adding ice as an ingredient;(vii) Other effective methods. COS: Inspector immediately instructed PIC to move to Two Door standing Cooler and insert ice as an ingredient to provide rapid cooling. Corrected On-Site. New Violation. 12B Violation of Code: [511-6-1.03(5)(g)] Observed employee involved in food preparation with multiple rings on every finger, with designs and diamonds. Except for a plain ring such as a wedding band food employees may not wear jewelry including medical information jewelry on their arms and hands while preparing food. COS: PIC encouraged employee to remove rings during inspection. Corrected On-Site. New Violation.
