Violation of Code: [511-6-1.03(2)(a)-(l)(n)(o)] PIC failed in their duties to monitor cold holding, hot holding, hand sink operation, date marking and food- contact surfaces. There must be a person in charge on the premises of the food service establishment at all times. The person in charge shall ensure compliance with food safety and management of facility. COS: PIC was educated on food safety and restaurant protocol during inspection and was encouraged to attend a Servsafe class, take and pass exam. Corrected On-Site. New Violation. 1-2B Violation of Code: [511-6-1.03(3)(b)] Observed no CFSM on staff. PIC stated he has taken and failed Servsafe exam, inspector recommended taking Servsafe class and exam. At least one employee that has supervisory and management responsibility and the authority to direct and control food preparation and service shall be a certified food safety manager who has shown proficiency of required information through passing a test that is part of an accredited program that conforms to the national standards for organizations that certify individuals. Facility must take CFSM exam within (30) Days. Repeat Violation. Correct By: 08/19/2022 2-2D Violation of Code: [511-6-1.07(3)(a)] Upon entrance; observed no approved hand soap or paper towels at employee hand wash sink. Each handwashing sink or group of two adjacent handwashing sinks shall be provided with a supply of hand cleaning liquid, powder, or bar soap. COS: PIC provided hand soap for employee hand wash sink. Corrected On-Site. New Violation. 4-2B Violation of Code: [511-6-1.05(7)(a)1] Observed microwave to have an accumulation of debris. Equipment of food-contact surfaces and equipment shall be clean to sight and touch. PIC to properly clean and sanitize microwave at COB. New Violation. Correct By: 07/21/2022 6-1A Violation of Code: [511-6-1.04(6)(f)] Observed raw, shelled eggs cold holding on a kitchen counter at 65F. Except during preparation, cooking, or cooling, or when time is used as the public health control, time/temperature control for safety food shall be maintained at 41°F (5°C) or below. COS: PIC was advised to discard out of temperature item. Corrected On-Site. New Violation. 6-1B Violation of Code: [511-6-1.04(6)(f)] Observed cooked beef hot holding on a kitchen counter at 107F. Except during preparation, cooking, or cooling, or when time is used as the public health control, time/temperature control for safety food shall be maintained at 135F. COS: PIC was encouraged to reheat beef to a temperature of at least 165F and hot hold in an approved hot holding unit at 135F. Corrected On-Site. New Violation. 6-2 Violation of Code: [511-6-1.04(6)(g)] Observed rice in a reach in cooler prepped over 24 hours (per PIC) with no date marking to indicate prep and discard date(s).Except when packaging food using a reduced oxygen packaging method, and except as specified in paragraphs 4 and 5 of this subsection, refrigerated, ready-to-eat, time/temperature control for safety food prepared and held in a food establishment for more than 24 hours shall be clearly marked to indicate the date or day by which the food shall be consumed on the premises, sold, or discarded, when held at a temperature of 41°F or below for a maximum of 7 days. The day of preparation shall be counted as Day 1. COS: PIC voluntarily discarded item. Corrected On-Site. New Violation. 11A Violation of Code: [511-6-1.04(6)(e)] Observed rice cooling on a counter (per PIC) at 99F with no method to ensure proper cooling is taking place. Per PIC, rice was in cooling process for 2 hours. Employees must use proper methods to rapidly cool time/temperature control for safety food, that are not held hot or are not for consumption within four hours, through daily oversight of the employees’ routine monitoring of food temperatures during cooling. COS: PIC was advised on proper cooling methods and created a water and ice bath for rice. Corrected On-Site. New Violation. 11D Violation of Code: [511-6-1.05(2)(k)] Observed no thermometer in a reach in cooler. In a mechanically refrigerated or hot food storage unit, the sensor of a temperature measuring device shall be located to measure the air temperature in the warmest part of a mechanically refrigerated unit and in the coolest part of a hot food storage unit. PIC to provide a temperature measuring device by COB. New Violation. Correct By: 07/21/2022 13A Violation of Code: [511-6-1.02(1)(d)] Observed last inspection failing to be posted in facility for public viewing. The most current inspection report shall be prominently displayed in public view at all times, within fifteen feet of the front or primary public door and between five feet and seven feet from the floor and in an area where it can be read at a distance of one foot away. COS: PIC posted most current inspection for public viewing. Corrected On-Site. New Violation. 15C Violation of Code: [511-6-1.05(7)(d)] Observed gasket on 2-door reach-in cooler to have an accumulation of debris. Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues. PIC to clean and sanitize gaskets by COB. Repeat Violation.
