Violation of Code: [511-6-1.05(7)(a)1] Ice machine with large build up of mold and residue on baffle. 1. Equipment food-contact surfaces and utensils shall be clean to sight and touch. PIC to properly clean interior of ice machine at COB. New Violation. Correct By: 11/22/2021 17C Violation of Code: [511-6-1.07(5)(a),(b)] Observed duct tape as a means to prevent leaking and/or hold faucet together on 3 compartment sink. Physical facilities shall be maintained in good repair Facility must correct within (3) days time. New Violation. Correct By: 11/24/2021 17D Violation of Code: [511-6-1.07(5)(d)] Observed an accumulation of dust on ventilation system above prep top cooler. Intake and exhaust air ducts shall be cleaned and filters changed so they are not a source of contamination by dust, dirt, and other materials. Facility must correct in (3) days time. New Violation. Correct By: 11/24/2021
