Violation of Code: [511-6-1.05(6)(n)] Observed all sanitizing buckets with 0-100 ppm of QAC. A quaternary ammonium compound solution shall have a concentration as specified in DPH Rule 511-6-1-.07(6)(g) and as indicated by the manufacturer’s use directions included in the labeling. COS: PIC remade sanitizer to a concentration of 200-400 ppm of QAC. Corrected On-Site. New Violation. 12B Violation of Code: [511-6-1.03(5)(g)] Observed employee involved in food preparation with an arm bracelet. Except for a plain ring such as a wedding band food employees may not wear jewelry including medical information jewelry on their arms and hands while preparing food. COS: Employee removed arm bracelet. Corrected On-Site. New Violation. 16A Violation of Code: [511-6-1.06(1)(g),(h)] Observed (2) hand sinks failing to meet minimum hot water requirement for a food establishment. Hot water generation and distribution systems shall be sufficient to meet the peak hot water demands throughout the food service establishment. Note: Hand sink water must reach a minimum temperature of 100 degrees Fahrenheit. [See Observations.] Facility must correct violation within (10) days time. Repeat Violation. Correct By: 10/03/2021 17C Violation of Code: [511-6-1.07(5)(a),(b)] Observed ceiling tiles missing and/or in bad condition through out facility. Observed gaskets on cold holding units with an accumulation of dark debris and food particles. Physical facilities shall be maintained in good repair. Facility must correct violation within (10) days time. New Violation. Correct By: 10/03/2021
