Violation of Code: [511-6-1.03(2)(a)-(l)(n)(o)] Observed a pattern of noncompliance and failure by the Person In Charge to ensure employee compliance. There must be a person in charge on the premises of the food service establishment at all times. The person in charge shall ensure compliance with Foodborne Illness Risk Factors and Public Health Interventions. COS: PIC was educated by inspector on Foodborne Illness Risk Factors, Public Health Interventions and Good Retail Practices during inspection. Corrected On-Site. New Violation. 4-2A Violation of Code: [511-6-1.04(4)(c)1(iv)] Observed sausage patties, raw chicken and waffle batter mix stored in reach in coolers with out protection from above. Food shall be protected from cross contamination by: storing the food in packages, covered containers, or wrappings. COS: PIC placed covers on all uncovered foods. Corrected On-Site. New Violation. 4-2B Violation of Code: [511-6-1.05(7)(a)1] Observed an accumulation of mold and slime in operating ice machine. Equipment food-contact surfaces and utensils shall be clean to sight and touch. COS: Ice machine thoroughly cleaned and sanitized during inspection. Corrected On-Site. New Violation. 8-2B Violation of Code: [511-6-1.07(6)(g)] Observed sanitizing buckets and 3 compartment sink water to have a concentration of 200 ppm of chlorine solution. Chemical sanitizers, including chemical sanitizing solutions generated on-site, and other chemical antimicrobials applied to food-contact surfaces shall: Meet requirements specified in 40 CFR 180.940 Tolerance exemptions for active and inert ingredients for use in antimicrobial formulations (Food-contact surface sanitizing solutions) COS: PIC diluted the chlorine solution with water to a concentration of 50-100 ppm. Corrected On-Site. New Violation. 8-2B Violation of Code: [511-6-1.07(6)(r)] Observed personal items being stored on top of a drink carbonation system and on shelving meant for dry storage. Facility failed to identify a personal storage area for belongings. Employees shall store their personal care items in lockers or other facilities for orderly storage. COS: PIC separated personal items and placed in an area for orderly storage. Corrected On-Site. New Violation. 13A Violation of Code: [511-6-1.02(1)(d)] Observed the most current inspection report not being prominently displayed for public viewing. The most current inspection report shall be prominently displayed in public view at all times, within fifteen feet of the front or primary public door and between five feet and seven feet from the floor and in an area where it can be read at a distance of one foot away. COS: PIC displayed most current inspection in a manner that meets regulations. Corrected On-Site. New Violation. 15C Violation of Code: [511-6-1.05(7)(d)] Observed an accumulation of dust on ventilation systems in restrooms. Observed shelving in Walk in Cooler to have an accumulation of debris. Observed floor tiling to have an accumulation of food and dirt debris. Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues.
