Violation of Code: [511-6-1.03(2)(m)] pic could not provide Employee Health documentation that shows employees have been trained on reporting symptoms and diseases that are transmissible through food. Food employees and conditional employees are informed in a verifiable manner of their responsibility to report in accordance with the Chapter to the person in charge, information about their health and activities as they relate to diseases that are transmissible through food. PIC was given a copy of the state Employee Health form. New Violation. Corrected On-Site. New Violation. 12B Violation of Code: [511-6-1-.03(5)(j)3] Observed manager with a big cut on his left arm exposed. Employees shall maintain a high degree of personal cleanliness and shall use good hygienic practices during all working periods in the food service establishment.
