Violation of Code: [511-6-1.06(2)(o)] Observed kitchen handwashing sink closest to the bun toaster and prep top cooler being made inaccessible by the placement of the bun shelving. A handwashing sink shall be maintained so that it is accessible at all times for employee use. COS: PIC moved the bun shelving out of the way to make the hand washing sink accessible. Corrected On-Site. New Violation. 4-2B Violation of Code: [511-6-1.05(6)(n)] Warewashing machine observed with 0ppm Cl. Per warewashing machine manufacturer data plate the machine shall operate with a minimum of 50ppm Cl. COS: PIC primed the machine and rewashed the dishes with a Cl concentration of 50ppm. Corrected On-Site. New Violation. 13A Violation of Code: [511-6-1.02(1)(d)] Observed no inspection report to be posted in the drive-thru window. Food service establishments with drive-thru windows will post the current inspection report, and also have the inspection report posted so that a minimum of the top one-third of a copy of the current inspection report is visible through each window allowing customers to easily read the score, date of inspection and establishment information. COS: PIC posted inspection report in drive-thru window. Corrected On-Site. New Violation. 17C Violation of Code: [511-6-1.07(5)(a),(b)] Observed ventilation hood and light located above the ovens used in the mornings to not be operating. The light did not turn on and the hood did not suction the paper towel used to test it. All physical facilities shall be maintained in good repair. PIC to have unit repaired within 10 days. New Violation. Correct By: 04/26/2021 18 Violation of Code: [511-6-1.07(5)(k)] Observed an excessive number of flies within the facility. The presence of insects, rodents, and other pests shall be controlled to minimize their presence on the premises by Routinely inspecting the premises for evidence of pests; Using methods, if pests are found, such as trapping devices or other means of pest control as specified under subsections (6)(e), (6)(m), and (6)(n) of this Rule. PIC to work with a PCO to control the number of flies in the facility and to supply Health Authority with an action plan within 10 days. New Violation. Correct By: 04/26/2021
