Violation of Code: [511-6-1.03(2)(a)-(l)(n)(o)] No PIC present upon beginning of inspection- no employees assigned the duty of ensuring food safety compliance in absence of a manager There must be a person in charge on the premises of the food service establishment at all times. Repeat Violation. 4-2B Violation of Code: [511-6-1.05(6)(n)] Observed employee use wiping cloth from sanitizing bucket to clean the top food contact surface at the front customer service area. When the sanitizer was tested it measured 0ppm of Quaternary Ammonium. A Quaternary Ammonium compound solution shall have a concentration as specified in DPH RULE 511-6-1-07(6)(g). The sanitizer solution was discarded and a new sanitizer solution that measured between 150 - 400ppm of Quaternary Ammonium was prepared. New Violation. Corrected On-Site. New Violation. 6-1A Violation of Code: [511-6-1.04(6)(f)] Observed Meatballs inside the Vegetable sink at a temperature of 57 Degrees Fahrenheit. Also observed ham in the reach in cooler at 50 Degrees Fahrenheit. TCS foods shall be maintained at 41 Degrees Fahrenheit or below or 135 Degrees Fahrenheit or Above. Both Meatballs and ham were voluntarily discarded during the inspection. New Violation. Corrected On-Site. New Violation. 12B Violation of Code: [511-6-1.03(5)(i)] Observed an employee with no hair restraint preparing food. All employees who handle food must wear and approved hair restraint.
