Violation of Code: [511-6-1.05(7)(a)1] Observed ice machine with an accumulation of slime mold. Food-contact surfaces shall be clean to sight and touch. PIC to empty, clean and sanitize ice machine at C.O.B. Repeat Violation. Correct By: 01/11/2021 Violation of Code: [511-6-1.05(2)(x)] Observed reach in cooler and reach in refrigerator to lack temperature measuring devices. All cold or hot holding equipment for time/temperature control for safety food shall include a temperature measuring device. PIC to equip all hot or cold holding equipment with temperature measuring devices. Correct w/in 10 days. New Violation. Correct By: 01/18/2021 Violation of Code: [511-6-1.07(5)(a),(b)] Observed ceiling tile buckling with hole, and an accumulation of mold. All physical facilities shall be maintained in good repair. PIC to have maintenance determine source of leak in roof/ceiling and repair w/in 30 days. New Violation. Correct By: 02/07/2021
