Violation of Code: [511-6-1.07(6)(b)] Observed multiple chemical spray bottles unlabeled. 1 spray bottle was filled with concentrated >200ppm chlorine sanitizer. COS: PIC diluted sanitizer and labeled each spray bottle. Corrected On-Site. New Violation. Violation of Code: [511-6-1.05(7)(a)2,3] Observed walk-in cooler racks and fan with an accumulation of dust and mold. Repeat violation. PIC to clean and sanitize walk-in cooler. Repeat Violation.
