Violation of Code: [511-6-1.04(4)(i)] Observed ice accumulation in contact with food packaging in walk-in freezer. Ice condensate from walk-in freezer must not accumulate on food or packaging. Walk-in freezer to be repaired and maintained free of ice accumulation. New Violation. Violation of Code: [511-6-1.05(6)(a)] Observed door seal and door itself of reach-in freezer next to fryer damaged. Replace broken seals. Repair or replace freezer door. Seals damaged on reach-in cooler across from freezer. Replace seals. Observed walk-in cooler floor damaged and cracked. Floor is unable to be mopped without water entering under floor. Repair or replace walk-in cooler floor. New Violation. Violation of Code: [511-6-1.07(2)(a)] Observed ceiling tiles missing in main kitchen. Replace ceiling tiles. New Violation. Violation of Code: [511-6-1.05(2)(n)] Observed tape and towels being used to collect grease on hood. The hood must be designed and maintained to collect grease in a way that prevents contamination. New Violation.
