Violation of Code: [511-6-1.03(3)(c)] CFSM posted Jonathan Brobston still no longer employed at this establishment. Food service establishments shall have in its employ a Certified Food Safety Manager (CFSM) as specified in paragraph to ensure food safety is being managed within the food service establishment during all hours of operation. PIC to remove Jonathan Brobston CFSM certificate from display. Establishment to either post current employee certificate if applicable or employ a CFSM within 60 days. Repeat Violation. 4-2B Violation of Code: [511-6-1.05(6)(n)] Observed lobby sanitizer bucket and front food service sanitizer bucket with 0ppm QAC sanitizer. QAC sanitizer must be maintained at a concentration according to the manufacturer (QAC usually 150 - 400ppm). COS: PIC replaced both sanitizer buckets with fresh sanitizer solution that measured 200ppm QAC. Corrected On-Site. New Violation. 12B Violation of Code: [511-6-1.03(5)(i)] Observed an employee with no hair restraint preparing food. All employees who handle food must wear and approved hair restraint. COS: employee switched tasks to run the register. Corrected On-Site. New Violation.
