Violation of Code: [511-6-1.05(7)(b)] Heavy buildup of mold in ice machine behind screwed in baffle. Food contact surfaces must be cleaned at a frequency necessary to prevent the accumulation of mold. COS: Employees scooped out ice and thoroughly cleaned the interior portions with a sanitized rag. Corrected On-Site. New Violation. 14A Violation of Code: [511-6-1.04(4)(k)] Ladles stored in room temperature water next to steam well and rice cookers. During pauses in food preparation or dispensing, food prep and dispensing utensils may be stored in a container of water if the water is maintained at a temperature of at least 135F. COS: Ladel removed and placed on clean portion of food prep surface. Knife blade stored in uncleaned slit area between makeline coolers. Store utensils only in a clean location that is protected from possible contamination. COS: Knife removed and placed with the dirty dishes to be washed Corrected On-Site. Repeat Violation. 15B Violation of Code: [511-6-1.05(6)(j)] Chemical sanitizing dish machine recorded at a maximum wash temperature of 111F. The wash temperature of chemical sanitizing dish machines must not be less than 120F. After CFSM adjusted hot water heater, dish machine still only up to 118F. CFSM informed to contact service provider for maintenance. New Violation. 15C Violation of Code: [511-6-1.05(7)(d)] Handles and insides of equipment, shelving throughout kitchen and above 3 compartment sink with build up of dust, grease, and food residue. Non-food contact surfaces shall be cleaned at a frequency to prevent the build up of accumulations. Repeat Violation
