<p>The following is a list of documents _ which must be originals or copies certified by the issuing agency _ accepted by the Georgia Department of Human Resources to verify citizenship and identity for all Medicaid applications. The department says it will use this list for adults applying for all non-emergency benefits.</p><p>For these documents, no additional form of ID is required:</p><p>-- U.S. passport</p><p>-- Certificate of Naturalization</p><p>-- Certificate of Citizenship</p><p>For these documents, another form of ID is required:</p><p>-- U.S. public birth record</p><p>-- U.S. birth certificate</p><p>-- Certification of Report of Birth</p><p>-- Consular report of birth abroad of a citizen of the U.S.</p><p>-- Certification of birth abroad</p><p>-- United States Citizen Identification Card</p><p>-- American Indian Card</p><p>-- Collective Naturalization document/Northern Mariana Identification Card</p><p>-- Final Adoption Decree</p><p>-- Evidence of civil service employment by the U.S. government</p><p>-- Official military record</p><p>-- Federal or state census record showing U.S. citizenship indicating a U.S. place of birth</p><p>-- Tribal census record for Seneca Indian tribe or from Bureau of Indian Affairs</p><p>-- Statement signed by the physician or midwife who was in attendance at the time of birth</p><p>-- One of the following documents created at least five years before the application for Medicaid showing a U.S. place of birth:</p><p>_Extract of hospital record on hospital letterhead established at the time of person's birth</p><p>_Life, health or other insurance record</p><p>_An amended U.S. public birth record</p><p>_Medical clinic, doctor or hospital record indicating a U.S. place of birth</p><p>_Institutional admission papers from nursing home, skilled nursing care facility or other institution</p><p>These are acceptable documents to verify identity, to be used with the documents above:</p><p>-- State driver's license or Georgia Identification Card for Voting Purposes</p><p>-- Certificate of Indian Blood, U.S. American/Alaska Native tribal document, Native American Tribal Document</p><p>-- U.S. Military Card or draft record; Military dependent's ID card with photograph; U.S. Coast Guard Merchant Mariner Card</p><p>-- Photo IDs issued by federal, state or local government agencies</p><p>-- School Identification card with a photograph</p><p>-- U.S. passport issued with limitations</p><p>Source: Georgia Department of Human Resources.</p>
