GAINESVILLE - Northeast Georgia Diagnostic Clinic (NGDC) recently received the highest level of Patient-Centered Medical Home (PCMH) Recognition by the National Committee for Quality Assurance (NCQA), part of a national initiative for improving care coordination in primary care. To achieve this recognition, the Clinic demonstrated compliance with NCQA’s standards which emphasize the use of systematic, patient-centered, coordinated care that supports access, communication, and patient involvement.
The NCQA Patient-Centered Medical Home is a model of primary care that combines teamwork and information technology to improve care, improve patients’ experience of care and reduce costs. Medical homes foster ongoing partnerships between patients and their personal physicians, instead of approaching care as the sum of episodic office visits. Each patient’s care is overseen by physician-led care teams that coordinate treatment across the health care continuum. Research shows that medical homes can lead to higher quality and lower costs, and can significantly improve patient satisfaction. NCQA is a private, non-profit organization dedicated to improving health care quality, and whose mission is to improve the quality of health care.
While many of the Clinic’s care enhancements are behind the scenes, there are some things patients will easily notice, including more access for same-day acute visits, a robust online patient portal, clinical summaries of your appointments, timely responses to requests, quick access to medical records, and the establishment of our comprehensive Care Management Team to further enhance comprehensive care that is focused on you.
