Violation of Code: [511-6-1.04(6)(f)] Observed mac & cheese hot holding in warmer cabinet in smoker enclosure at 125F. Time/temperature Controlled for Safety foods must be hot held at 135F or higher to prevent the growth of potentially harmful bacteria. COS: PIC reheated mac & cheese to 165F within 15 minutes. All hot holding units must be adjusted to hold food at 135F or higher. Corrected On-Site. Repeat Violation. 15A Violation of Code: [511-6-1.05(6)(a)] Observed ice and water accumulation on floors, walls, and ceilings in walk-in freezer. Non-potable water is dripping and freezing onto food packages. Ensure no food is stored under dripping water. All foods in walk-in cooler must be protected from contamination. Have walk-in freezer repaired. Repeat Violation. 17C Violation of Code: [511-6-1.07(5)(a),(b)] Observed large hole in wall behind mop sink in main kitchen. Observed multiple tiles missing in wall next to mop sink. Mop sink wall to be repaired as soon as possible. Repeat Violation. Remarks Ensure all hot holding units are set to hold food at 135F or higher. Ensure all hot holding units are maintained and capable of holding food at 135F or higher
