Violation of Code: [511-6-1.03(5)(j)1&2] Observed employee water bottles and travel mug in main kitchen on rack above prep table. Drinks may be kept in the kitchen in a single service cup with lid and straw. Drinks must be kept where they cannot contaminate food or equipment. PIC to train employees on the need to use single service cups with lids and straws in the kitchen. New Violation. 2-2D Violation of Code: [511-6-1.06(2)(o)] Observed employee scrubbing knife in grill area handsink. Handsinks are for hand washing only. All dishes and utensils are to be cleaned in the 3 compartment sink or dish machine. COS: Employee took knife to be cleaned in dish machine. Corrected On-Site. New Violation. 4-2B Violation of Code: [511-6-1.05(6)(n)] Observed sanitizer bucket in drive-thru area with 0ppm QAC sanitizer concentration. QAC sanitizer must have a concentration according to the manufacturer (usually 200 - 400ppm). COS: Employee remade sanitizer at 200ppm. Corrected On-Site. New Violation. 4-2B Violation of Code: [511-6-1.05(7)(b)] Observed dining room tables being cleaned with soap and water only. Dining counters and table-tops shall be cleaned and sanitized routinely after removing all soiled tableware and food trays shall be cleaned and sanitized after each use by one of the following methods: (i) A two step method in which one cloth, rinsed in sanitizing solution is used to clean food debris from the surface and a second cloth in separate sanitizing solution is used to rinse (ii) Sanitizing solution is sprayed onto the surface and the surface is then wiped clean with a disposable towel COS: Employees filled up container with sanitizer solution and towels for wiping dining area tables. Corrected On-Site. New Violation. 4-2B Violation of Code: [511-6-1.05(7)(a)1] Observed rolling pin stored in dry storage area uncleaned. Ensure dishes and utensils are cleaned before storage. COS: PIC had rolling pin cleaned before storing. Corrected On-Site. New Violation. 6-1B Violation of Code: [511-6-1.04(6)(f)] Observed pot roast holding in warmer cabinet just outside walk-in freezer at 130F. Observed cooked broccoli holding at 123F in same cabinet. Time/temperature controlled for safety foods must be hot held at 135F or higher. PIC states both pot roast and broccoli had been prepared and placed in cabinet around 10:45AM. COS: Employee reheated broccoli and pot roast to greater than 165F within 10 minutes (see log). PIC turned warmer unit from setting 4 to setting 9. Corrected On-Site. New Violation. 6-1C Violation of Code: [511-6-1.04(6)(d)] Observed vegetable soup that had been cooked and placed in walk-in cooler at 7:30PM 1/21/20. Vegetable soup was holding at 54F. Time/temperature controlled for safety foods must be cooled from 135F to 70F within 2 hours and from 70F to 41F within 4 hours after that. Vegetable soup did not cool to 41F within 6 hours. Food that are cooled improperly have an increased risk for harmful bacterial growth. COS: PIC voluntarily discarded vegetable soup. Corrected On-Site. New Violation. 12A Violation of Code: [511-6-1.04(4)(q)] Observed multiple jugs of tea being stored on floor next to drink machine in dining area. All food must be stored at least 6 inches off the floor. COS: PIC placed tea jugs on countertop. Corrected On-Site. New Violation. Violation of Code: [511-6-1.05(4)(b)] Observed main kitchen handsink, grill area handsink, and men's restroom handsink unsealed to adjacent walls. Fixed equipment that is exposed to moisture must be sealed to adjacent walls. Sinks to be resealed to walls. New Violation. 15A Violation of Code: [511-6-1.05(1)(a)] Observed duct tape being used to repair plastic scoop in sugar bin. Observed duct tape being used to hold up gasket on ice machine door. Do not use duct tape on surfaces that will or could contact food. Replace scoop in sugar bin. Replace gasket in ice bin. New Violation. 15A Violation of Code: [511-6-1.05(6)(a)] Observed ice accumulation and leaking from fan of walk-in freezer. This is non-potable water and may contaminate food. Ensure no food is stored under dripping water. Place pan in areas subject to leaking. Have walk-in freezer repaired. New Violation. 17C Violation of Code: [511-6-1.07(5)(f)] Observed multiple mop heads stored on floor in alcove next to bathrooms. Mops must be stored in a manner that allows them to air dry. PIC to hang up mops on already installed mop hangers. New Violation. 17C Violation of Code: [511-6-1.07(5)(a),(b)] Observed large hole in wall behind mop sink in main kitchen. Observed multiple tiles missing in wall next to mopsink. Mop sink wall to be repaired immediately. New Violation.
