Violation of Code: [511-6-1.03(3)(a)] Establishment had no CSFM certification posted in public view in food service establishment. Original copy of CSFM had expired 5/29/18. A CSFM certificate which has expired been revoked or suspended shall not be posted in food service establishment. New Violation. 17B Violation of Code: [511-6-1.06(5)(n)] Observed dumpster receptacle with open side doors. Dumpsters should be kept closed and tightly shut to avoid possible insect attraction and rodent entry. New Violation.
