Violation of Code: [511-6-1.03(5)(c)] Employee working the line not washing hands between changing gloves to perform different tasks. Employee began cleaning with the sanitizer bucket, before attempting to work with ready to eat foods. Employees must wash hands between different tasks, and when changing gloves to perform a different task, or the same task, if hands become contaminated. COS: Employee instructed to wash hands between glove change before continuing on with task. Corrected On-Site. New Violation. 2-2D Violation of Code: [511-6-1.07(3)(a)] Front counter handsink soap dispenser no longer mounted to wall and empty. In lieu of soap dispenser, no alternative cleanser is provided. All handsinks in a facility must be equipped with cleanser to allow employees to clean hands. New Violation. 2-2E Violation of Code: [511-6-1.03(6)] Facility does not have either written procedures or acceptable chemicals/equipment required for the clean up of contamination events such as vomit or diahreaa. All food service establishments must have these in order to prevent contamination of foods, employees, and equipment. COS: Provided establishment with written clean up procedures. Health department recommends establishment obtain a clean up kit. Corrected On-Site. New Violation. 6-1A Violation of Code: [511-6-1.04(6)(f)] Cheeses, and ranch kept in pans on top of separate pans of ice along food prep area recorded at ranges of 53F-57F (see log). Time and temperature control for safety foods must be cold held at 41F or below. COS: Foods just recently placed onto ice bath. Ice bath refilled and containers submerged directly into ice. Corrected On-Site. New Violation. 6-1B Violation of Code: [511-6-1.04(6)(f)] Sauteed mushrooms recorded at 93F, and sauteed onions recorded at 113F-145F on stove top. Time and temperature control for safety foods must be hot held at 135F or above. COS: Foods voluntarily discarded. Corrected On-Site. New Violation. 8-2B Violation of Code: [511-6-1.07(6)(e)] Quat ammonium based sanitizer kept in buckets recorded at above 400 PPM. Manufacter's direction in label specify that concentration must remain between 200-400 PPM. COS: Quat solution discarded. New solution made that was 400 PPM. Corrected On-Site. New Violation.
