Violation of Code: [511-6-1.03(5)(c)] Observed employees who didn't wash hands between changing gloves or changing task. Employee didn't wash hands or use gloves before or after handling dishes by dish machine. Employee also handling/cooking raw chicken wings went straight to a prep table and prepped a salad. Employees should wash hands and change gloves before, and after changing task in a food establishment. COS: Employee removed gloves and washed hands before continuing current task. Corrected On-Site. New Violation. 4-2B Violation of Code: [511-6-1.05(7)(b)] Observed mold within ice machine located on the ceiling, and the inside of drink dispenser nozzles. Food contact surfaces should be cleaned frequently enough to preclude mold/debris build up. New Violation. 6-1A Violation of Code: [511-6-1.04(6)(f)] Observed ground sausage, and grilled chicken with temperatures ranging from 45F-46F located within prep top cooler. TCS foods should be held at 41F or below to slow down/stop bacteria growth. COS: PIC discarded all foods that were held above 41F New Violation. 6-2 Violation of Code: [511-6-1.04(6)(g)] Observed Ham, ground sausage, tortellini, located within reach-in cooler without proper date labels. Dates ranging back from 3/23-3/29 and dates with discard date as 4/1- 4/4. Foods that are prepped should have a date label shelve life of 7 days with day 1 being the day that is prepped. COS: PIC removed all foods with improper date labeling. Corrected On-Site. New Violation. 12C Violation of Code: [511-6-1.04(4)(m)] Observed various cloths around establishment not fully submerged within sanitizer solution. Store sanitizer clothes in solution to reduce bacterial growth. COS: PIC stored all clothes within sanitizer buckets. Corrected On-Site. New Violation. 15B Violation of Code: [511-6-1.05(3)(h),(i)] Observed no QAC test strips within facility used for testing sanitizing solution in buckets. Establishment should have test strips to ensure solution has proper ppm. New Violation. 17B Violation of Code: [511-6-1.06(5)(q)] Observed dumpster receptacle with built up debris. Dumpster areas should be kept clean and at a frequency to keep debris built up and also keep insect/rodent entry. New Violation.
