Violation of Code: [511-6-1-.04(7)(e)] No asterisk provided for reminder statement on breakfast menu to indicate consumer advisory for eggs. Hamburger on dinner menu offered cooked to order but no disclosure or reminder statement for consumer advisory is present. Foods offered raw or undercooked must have a consumer advisory with a disclosure asterisk indicating the raw food offered and reminder asterisked with footnote in all capital letters with applicable advisory. COS- PIC began printing labels to correct existing menus and to place order for reprinted menus. Corrected On-Site. New Violation. 12A Violation of Code: [511-6-1.04(4)(h)] Opened bottle of sparkling water observed submerged in ice bin behind bar where ice is used for customer beverages. Ice may not be used as food after it has been in contact with bottled beverages. COS- PIC discarded all ice, cleaned and sanitized bin, and to discuss with bar staff proper practices for storing beverages and handling ice. Corrected On-Site. New Violation. 17B Violation of Code: [511-6-1.06(5)(k),(l)] Dumpster has missing top lid on left side and rain plug damaged with large hole. Enclosure area has accumulated leaves and debris. Dumpsters and surrounding enclosures must be kept maintained and in good repair. GM to contact dumpster company for replacement and have staff clean area. Repeat Violation.
