Violation of Code: [511-6-1.03(1)(c)2,3] Employees do not have enough training on responsibility to report to the person in charge information about their health as it relates to illness most transmissible through food. Employees could not list any symptoms or illness. COS: Provided establishment with educational material for reference. Corrected On-Site. New Violation. 3-1A Violation of Code: [511-6-1.04(2)(a)] When PIC began explaining how salsa verde cooling in merchandiser unit was prepared, he mentioned that they picked it up this morning from home. Food prepared in a private home or received from a consumer may not be used or offered for human consumption is a food service establishment. COS: Informed PIC that food prepared from home cannot be served in establishment. Salsa removed from establishment. Corrected On-Site. New Violation. 11D Violation of Code: [511-6-1.05((2)(l)] Merchandiser unit missing ambient thermometer. Without probe, it is difficult to assess the temperature of unit currently cooling multiple TCS foods. New Violation. 17C Violation of Code: [511-6-1.07(5)(a),(b)] Grease hood heavily built up with oils and grease. Physical facilities must be maintained in good repair and cleaned frequently enough to prevent build up of accumulations. New Violation.
