Violation of Code: [511-6-1.03(3)(c)] CFSM certificate for Cynthia Brown expired October 2015. Within 90 days of CFSM certification expiration, the CFSM shall enroll in an approved training course, pass the exam, and obtain a new certificate. Establishment has 30 days to submit prof of enrolment. Hall Co. Environmental Health 770-531-3973 offers a February ServSafe course as an enrollment option. Repeat Violation. 14A Violation of Code: [511-6-1.04(4)(k)] Tongs (not yet used) for house salad stored with handle portion contacting lettuce. Scoop for powdered sugar has no designated handle portion and stored submerged in product. In-use utensils must remain stored with their handles above the top of the food container or above the top of food within containers such as closed bins of dry foods. COS- PIC voluntarily discarded powdered sugar scoop and relocated handle portion of tongs above lettuce. Corrected On-Site. New Violation. 14B Violation of Code: [511-6-1.05(10)(e)1,2,4] Accumulated food debris and residues along crevices of clean knife rack and along ridges of rubber mats lining shelves for clean utensils. Clean utensils must be stored protected from splash, dust, or other sources of recontamination. PIC to remove knife rack from wall and rubber mats from shelving for cleaning/sanitizing by end of day and to incorporate a routine schedule for surfaces used for storing clean utensils. New Violation. 15A Violation of Code: [511-6-1.05(2)(a)] Pressed particle board shelves with laminate at expo cookline peeling along seams and corners where surface can no longer be easily cleaned and sanitized. Equipment must be designed and constructed to be durable and retain their characteristic qualities under normal use conditions- PIC to replace shelves with new material designed to withstand such use. New Violation. 15C Violation of Code: [511-6-1.05(7)(d)] Interior of reach-in cooler and walk-in cooler, including: shelving, fan guards, ceiling, walls, and wall attachments have accumulated dust and debris. Clean surfaces frequently and thoroughly enough to preclude accumulations. New Violation. 17C Violation of Code: [511-6-1.07(2)(c)] Covebase damaged or missing along portions of floor/wall juncture at cookline. In process of replacing covebase as floor tiles are repaired per PIC. New Violation. 17C Violation of Code: [511-6-1.07(2)(f),(g)] FRP torn and missing at water supply line under handsink at cookline exposing porous cinderblock subwall. Had plumbing issue below sink that has since been repaired per PIC. Walls and wall attachments must be finished and sealed to provide a smooth, nonabsorbent, and easily cleanable surface. PIC to have wall repaired to permitted condition. New Violation. 17D Violation of Code: [511-6-1.05(3)(d)] Accumulated grease and smoke staining walls and ceiling surrounding hood system at cookline. Ventilation hood systems and devices shall be sufficient in capacity to prevent grease build-up on walls and ceilings. PIC to contact professional service provider to assess mechanical components of system for adequate exhaust sizing and specifications and to submit invoice with corrective action recommendations to the Health Authority within 30 days. New Violation. Violation of Code: [511-6-1.07(5)(d)] Hood exhaust system has accumulated grease and charred debris running down sloped roof and on exterior of building where discharges to the ground near back service door. Intake and exhaust air ducts must be cleaned and filters changed at a frequency so that they are not a source of contamination. Ventilation systems to the outside must not create a public health hazard, nuisance, pest attraction, or unlawful discharge. PIC to have ventilation system evaluated by professional service provider and to submit invoice with corrective action recommendations to the Health Authority within 30 days. New Violation.
