Violation of Code: [511-6-1.03(2)] PIC not present. There must be a designated PIC on the premises of the food service establishment at all times- cook assigned as interim PIC for the purpose of the inspection. COS- CFSM arrived during inspection. Corrected On-Site. New Violation. 2-1B Violation of Code: [511-6-1.03(5)(c)] Observed employee returning to kitchen from resident area don gloves to prepare foods without first washing hands. Observed employee handling soiled utensils don gloves to handle clean utensils without first washing hands. Employees must wash hands after handling soiled equipment/utensils and immediately before engaging in food preparation including working with exposed food, clean equipment and utensils, and before donning gloves to initiate a task that involves working with food. COS- Discussed with employees, employees washed hands and donned gloves before engaging in new clean utensil and food handling tasks. Corrected On-Site. New Violation. 4-2B Violation of Code: [511-6-1.05(6)(n)] No sanitizer registering on test strips for quat sanitizer bay of 4 compartment sink or quat dispenser. Quat sanitizing solution must be used according to manufacturers required concentration (150-400ppm). COS- dispenser/sink reconfigured, now dispensing at 400ppm. Corrected On-Site. Repeat Violation. 6-1A Violation of Code: [511-6-1.04(6)(f)] Shredded iceberg lettuce stored in walk-in cooler 50F. TCS foods must maintain 41F or less for cold-holding. COS- Iceberg voluntarily discarded. Cold holding capacity/requirements discussed with district director of services group, coordinating course of corrective action. To discontinue use of walk-in until resolved. Repeat Violation. 6-1B Violation of Code: [511-6-1.04(6)(f)] Chicken hot held on steamtable 89-127F. TCS foods must maintain 135F or more for hot holding. COS- Cook demonstrated chicken was prepared less than 2 hours ago, chicken reheated to above 165F on stovetop before serving. Corrected On-Site. New Violation. Remarks Ice machine must be relocated to kitchen to ensure ice has protection from potential contamination and indirect drainage.
