Violation of Code: [511-6-1.03(3)(c)] Expired CFSM Certificate posted. Certification expires March 2017. The ninety days given after the expiration of a certification to enroll in a training course and pass an approved exam to obtain new certification has expired. Must show proof of registration for re-certification within 10 days (either registering for course and exam or just exam for recertification). New Violation. Correct By: 11/18/2017 2-2A Violation of Code: [511-6-1.03(1)(c)2,3] PIC not able to describe common symptoms of disease the are transmissible through food. Discussed symptoms- vomiting, diarrhea, jaundice, sore throat and fever and skin infections such as open sores (especially on hands or lower arms. New Violation. 2-2A Violation of Code: [511-6-1.03(2)(m)] PIC had no proof of a verifiable means by which employees are informed of their responsibility to report to the person-in-charge symptoms of illnesses that may be transmissible through food and/or if they have any illness known to be transmissible through food and/or have been exposed to anyone with these illnesses. Provided PIC with "Georgia Conditional Employee or Food Employee Reporting Agreement" which can be used for training and signed to meet the verifiable requirement. New Violation. 2-2E Violation of Code: [511-6-1.03(6)] PIC not aware of how to respond to a vomit or diarrheal event should it occur in the food service establishment. Procedure must be established for employees to follow in the event such an incident should occur which shall address actions employees must take to minimize the spread of contamination and the exposure of employees, consumers, food and food contact surfaces. Left copy of "Clean-up Procedures for Vomit/Fecal Events" provided by DPH for review with employees. Can post on wall as reminder for employees. New Violation. 11D Violation of Code: [511-6-1.05(3)(g)] No food temperature measuring device could be located in the kitchen. Must have a thermometer available for monitoring cooked, reheated and hot and cold food temperatures. Need to get one before events this weekend. New Violation. 15B Violation of Code: [511-6-1.05(3)(h),(i)] Could not locate test strips for monitoring the strength of sanitizer used in the 3-compartment sink for sanitizing kitchenware/utensils/equipment and in sanitizer buckets used to store cloths used to wipe food contact surfaces between uses. Using automatically dispensed QUAT sanitizer - need QUAT test strips. Locate or purchase before events this weekend. New Violation. 18 Violation of Code: [511-6-1.07(5)(k)] Evidence of rodent activity in old walk0in cooler that had until recently been used for dry storage. Source of activity could have been from some old trailers moved from location close to restaurant. To control more entry, must control entry into the facility and contact licensed pest control operator to help control activity inside food service establishment. New Violation. Remarks The only food service during the week is French fries and frozen wings (uncooked fried in deep fryer) prepared if customers come in. Food brought in for preparation on weekends for parties/events.
