Violation of Code: [511-6-1.07(5)(a),(b)] Drain cover on floor in front of fryers has large opening with sharp corners exposed. Physical facilities must be maintained in good repair. New Violation. Correct By: 12/06/2017 17D Violation of Code: [511-6-1.07(4)(b)] Employee cell phones lying on rack of clean equipment and single use articles. Lockers or designated areas for employee personal items must be located where contamination of food, equipment, utensils, linens, and single-use articles cannot occur. COS: Cell phones moved to cabinet for employee personal items. Corrected On-Site. New Violation. 17D Violation of Code: [511-6-1.07(2)(i)] Fluorescent light bulbs without a protective cover in room that houses ice machine and also unwrapped single-use articles. Light bulbs must be shielded in areas where potential contamination of food, equipment, utensils, linens and single-service articles can occur with either tubing or cover for bulbs. New Violation.
