Violation of Code: [511-6-1.03(2)(m)] Food employees have not been informed in a verifiable manner of their responsibility to report to the person in charge information about their health and activities as they relate to diseases transmissible through food. Discussed what is needed with CFSM and left a copy of the Georgia Conditional Employee or Food Employee Reporting Agreement with CFSM. New Violation. 12B Violation of Code: [511-6-1.03(5)(g)] Food handler wearing bracelet while prepping/cooking food. The only jewelry allowed to be worn by food handlers on hands or lower arms is a plain ring such as wedding band. COS: Food handler removed bracelet. Corrected On-Site. New Violation. 12B Violation of Code: [511-6-1.03(5)(i)] Food handlers not wearing hair restraint - a hat/hair net worn to properly restrain loose hair is required to be worn by employees preparing or handling food. New Violation. 15C Violation of Code: [511-6-1.05(7)(d)] Many cooler/drawer handles, outsides of equipment and gaskets with build-up of food debris. The outsides of equipment and gaskets need to be cleaned frequently enough to prevent build-up. Repeat Violation. 17B Violation of Code: [511-6-1.06(5)(q)] Recycle grease container with build-up of grease on top of container and on ground in front of the container. This can be an attraction for insects/rodents - clean spills to prevent build-up. New Violation.
