Violation of Code: [511-6-1.05(7)(b)] Soda dispensing nozzles had visible mold accumulations. All food contact surfaces must be cleaned at a frequency that is adequate to preclude accumulations. COS: Manager turned soda machine off and cleaned nozzles. Corrected On-Site. Repeat Violation. 15B Violation of Code: [511-6-1.05(6)(h),(i)] Quaternary test strips were water damaged. Replace damaged strips with new product so the sanitizer concentration can be correctly maintained. Repeat Violation. 15C Violation of Code: [511-6-1.05(7)(d)] Bottom of cold holding units, wall above sink and where sink is attached to wall with mold and food debris. Equipment must be cleaned at a frequency that is adequate to preclude accumulations. Repeat Violation. 17A Violation of Code: [511-6-1.07(5)(h)] Provide a covered receptacle in the women's restroom. COS: Person in charge made note to add cover. Repeat Violation. 17B Violation of Code: [511-6-1.06(5)(r),(s)] Dumpster area with trash on ground all around dumpster-grease container with door open and grease spilled all over container and on ground. Clean entire area to prevent insect/rodent attraction. New Violation.
