Violation of Code: [511-6-1.03(2)(m)] No verifiable way to confirm that employees have been trained in their responsibility to report to the person in charge information about their health as it relates to diseases that are most transmissible through food. COS: Provided establishment with an agreement reporting form. Employees need to fill it out with a signature and forms kept on file. Corrected On-Site. New Violation. 2-2E Violation of Code: [511-6-1.03(6)] Chemical establishment is currently using for cleaning up vomit/diarrheal contamination events is not EPA approved to kill norovirus. For contamination involving the aforementioned fluids, a disinfectant must be EPA approved to kill norovirus. COS: Recommended use of EPA approved bleach in future event of accident. Provided educational materials. Corrected On-Site. New Violation. 4-2B Violation of Code: [511-6-1.05(6)(n)] Bleach sanitizing solution buckets registering at 200+ PPM. Bleach sanitizer must be maintained at 50-100 PPM to safely and effectively be used as a food contact sanitizer. COS: Water added to sanitizing buckets. Corrected On-Site. New Violation. 4-2B Violation of Code: [511-6-1.05(6)(n)] Chemical sanitizing dish machine not producing any sanitizer after repeated cycles. Machine needed dip tube to be submerged into sanitizer source and for line to be primed. Maintain chlorine sanitizer at 50-100 PPM. COS: Dip tube submerged and line primed. Corrected On-Site. New Violation. Remarks This is a change in ownership for a food service permit. Rules and Regulations for Food Service for the State of Georgia require a Certified Food Safety Manager (CFSM) be on staff within 60 days of permitting. Owner is certified. Permit issued.
