Violation of Code: [511-6-1.03(3)(c)] Certificate posted for Richard Walters CFSM showing an expiration date of 6/16/17. A CFSM certificate which has expired must not be posted in the food service establishment. New Violation. 1-2B Violation of Code: [511-6-1.03(3)(b)] No verifiable way to confirm that an employee with supervisory experience is currently certified as a food safety manager. Certificate for Richard Walter expired and no certificate on file to back up PIC's claim that a manager is certified. Establishment must notify the health authority within 30 of the following measures taken to designate a new CFSM: 1)Hire a new CFSM 2)Enroll an existing employee in an approved CFSM training course; or 3)Hire a new employee who is currently enrolled in an approved CFSM training course New Violation. 6-2 Violation of Code: [511-6-1.04(6)(g)] Bag of chicken breasts in lowboy across from grill presumed to be opened on Sunday night do not bear a date label. Styrofoam containers of BBQ in same lowboy, typically kept for 2 or more days also did not contain a date label. Ready to eat Time/temperature control for safety foods must be labeled with a maximum discard date of 7 days from time of opening/prepping to control for microbial growth. COS: Containers were verified on date of opening and date label was applied Corrected On-Site. New Violation. 12A Violation of Code: [511-6-1.04(4)(q)] 3 buckets of icing stored directly on dirty floor of walk in cooler. Food must be stored at least 6 inches off the floor to prevent possible contamination. COS: Icing moved on top of shelf Corrected On-Site. New Violation. 13A Violation of Code: [511-6-1.02(1)(d)] Inspection report displayed was not the most recent. This is the second routine inspection this prior inspection (3/30/16) has been the one displayed. No inspection score posted in drive thru window either. The most current inspection report must be displayed at all times for public viewing in both the dining areas and at least the top third in the drive thru window. Repeat Violation. 15C Violation of Code: [511-6-1.05(7)(a)2,3] Heavy build up of encrusted grease and food residue on back half of grill and both fryers. Nonfood contact surfaces of equipment must be kept free of dust, dirt, food residue, grease, and other debris. New Violation. 17C Violation of Code: [511-6-1.07(5)(a),(b)] Floor inside walk in cooler found to contain in one corner heavy buildup of sludge debris, along corners and bottoms of walls food waste and slime. Physical facilities must be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products. New Violation.
