Violation of Code: [511-6-1.03(2)(m)] PIC has not informed employees in any manner - required to be in a verifiable manner - of their responsibility to report to the person in charge , information about their health as it relates to diseases transmissible through food. COS: Discussed this responsibility with the PIC and left form for PIC to go over with employees and employees can sign. Corrected On-Site. Repeat Violation. 4-2B Can opener with dried food debris build up around blade and inner parts. Ice machine has large build up of mold on inside of chute. Clean these items as frequently as necessary to not accumulate build up of mold or food debris. Repeat Violation. 6-1B Violation of Code: [511-6-1.04(6)(f)] Lo Mien with cooked cabbage left on shelf above prep cooler to be reheated for buffet with temperature of 77F. If this item is to be put out on food bar, must be prepared at time it is needed or it must be hot held at 135F or above until needed on buffet. COS: voluntarily discarded. Corrected On-Site. New Violation. 6-1D Violation of Code: [511-6-1.04(6)(i)] PIC explained some items on buffet are being held for time and not temperature. Must have written procedure if Time is being used to hold time and temperature control for safety food items instead of temperature. Must present written procedure to Health Authority within 10 days. New Violation. 11A Large plastic tub of rice covered and put into cooler to cool not cooling fast enough to meet required cooling parameters. To cool as rapidly as required, rice into smaller containers and leave uncovered and/or put rice in ice bath. COS: Owner placed rice in rice cooker to reheat and keep hot to use for fried rice. Corrected On-Site. New Violation. 12A Violation of Code: [511-6-1.04(4)(q)] Food in freezer stored on floor. All food must be stored at least six inches off the ground to prevent contamination. New Violation. 15A Condensate leak in 2-door sandwich unit. Pooled water in bottom of unit and hole drilled in bottom to allow water to accumulate in a pan underneath the unit. This water is considered nonpotable and should not be allowed to accumulate in this unit. Heavy ice build-up in walk-in freezer. This unit needs immediate defrosting. Repeat Violation.
