2-2A Violation of Code: [511-6-1.03(2)(m)] Certified food safety manager (PIC) had no verifiable proof of employees having knowledge of employees responsibilities having to report to PIC about health and activities as they relate to transmissible food born illnesses. New Violation. 2-2B Violation of Code: [511-6-1.03(5)(j)1&2] Disposable cups with lids and straws, bottled drink in reach in cooler and personal items found on cutting board, next to active prep station for onions, and near prep top tables. Drink in disposable lid and straws are allowed in the kitchen, however all personal drinks should be stored away from food contact surfaces to prevent contamination. New Violation. 2-2E Violation of Code: [511-6-1.03(6)] Facility had vomit kit intact, but didn't have a chemical included. No chemical found on site that is able to kill norovirus. All facilities should have a vomit procedure and kit in place incase a vomiting or diarrheal event were to occur. Can use bottle of Clorox/bleach - 1/3 cup bleach per 1 gallon water for nonporous surfaces. New Violation. 3-1C Violation of Code: [511-6-1.04(1)] Uncovered containers of breaded chicken within coolers with active dripping on them. Frozen condensation and icicles found on breaded chicken. Condensate is considered nonpotent water (not drinkable) and should not be able to come in contact with food/food surfaces. COS: Person in charge disposed breaded chicken. New Violation. 4-2B Violation of Code: [511-6-1.05(7)(b)] Ice machine and drink nozzles need adequate and more frequently cleaned. Accumulation of mold and debris on drink nozzles and inside ice machine door bin lid and baffle. Need brush to clean soda machine nozzles. New Violation. 6-1A Sunday topping cooler, and prep top cooler not holding cold holding temperatures. Prep top stocked to high to hold cold holding temperature. Sunday topping cooler needs to stay covered to keep temperature for whipped topping. 2nd Sunday Cooler not maintaining any temperatures below 41F. Ambient temperatures above 50F for 2nd Sunday topping. COS: Whip Cream No TCS food storage in the cooler until repaired. Corrected On-Site. New Violation. 15A Violation of Code: [511-6-1.05(2)(h)] Floor of walk-in freezer buckling so that it is not smooth and cleanable. Floor junctures need to be refastened so that floor can be easily cleaned and so that food debris do not accumulate under equipment. Repeat Violation. 15A Violation of Code: [511-6-1.05(2)(a)] Freezer, and cooler with condensation buildup and frozen condensate accumulation on freezer floor. Equipment must in good repair and maintained. Condensate must not present a hazard for employees and must not be able to accumulate on foods. Both condensate leak and repairs must be done within 10 days. Repeat Violation. Correct By: 09/02/2017
