Violation of Code: [511-6-1.05(7)(a)1] Many of the utensils and lexan containers on air drying rack with wet and dried food debris on them. After cleaning and sanitizing recheck for cleanliness before storage. COS: CFSM took all utensils, sheet pans and lexan containers to soak in 3-compartment sink before re-washing and sanitizing. Will discuss proper dishwashing with crew members. Corrected On-Site. Repeat Violation. 10D Violation of Code: [511-6-1.04(8)(b)] 6 half gallons of buttermilk under biscuit station found with best by date of 9/19/2017. 4 boxes (24 half gallons) of buttermilk in walk in with best by date of 9/19/2017. Refrigerated TCS foods cannot be used after the manufacturer best by, or sell by date. COS: Buttermilk voluntarily discarded Corrected On-Site. New Violation. 12A Violation of Code: [511-6-1.04(4)(q)] Mayo, biscuit mix boxes found directly on floor near the break area, and wire storage racks. Chicken filet box also found directly on floor in walk in freezer. Food must be protected from contamination by storing at least 6 inches off the floor. COS: Storage was re arranged to accommodate extra boxes. Milk crates also used to move food off of floor near break area. Chicken filets in freezer placed on shelves. Corrected On-Site. New Violation. 14B Violation of Code: [511-6-1.05(10)(a)] Most of the sheet pans and lexan containers stacked wet after cleaning and sanitizing - must allow kitchenware, utensils and equipment to air dry before stacking (storage). Repeat Violation. 14C Violation of Code: [511-6-1.05(10)(e)1&3] Boxes of single service items, such as plates and togo containers on floor near wire racks around the break area. Single service articles must be kept at least 6 inches off the ground. COS: Boxes moved on top of shelves and milk crates Corrected On-Site. New Violation. 17B Violation of Code: [511-6-1.06(5)(o),(p)] Left dumpster facing receptacle has a broken drain plug. Hole would allow for insect entry into dumpster and allow liquid waste to drip. Get a plug for drain hole to seal opening completely. Repeat Violation. 17B Violation of Code: [511-6-1.06(5)(o),(p)] Bags of trash found lying on ground outside of dumpster. Dumpster enclosures must be maintained to prevent the accumulation of trash, and debris. New Violation. 17C Violation of Code: [511-6-1.07(5)(a),(b)] Floors, coving, walls, tops of paper towel dispensers, and handsinks with heavy accumulation of flour and dried food debris. Physical facilities must be cleaned at a frequency to keep them free of accumulations New Violation.
