Violation of Code: [511-6-1.03(1)(a)-(c)] Owner (PIC) of facility not able to adequately answer questions regarding food safety procedures. PIC has 3 options for demonstrating knowledge. Not able to describe symptoms associated that are transmissible through food and or diseases. PIC does not meet any of these - Not Certified Food Safety Manager Has violations of priority items on inspection. Can not correctly respond to questions regarding public health and procedures. Repeat Violation. 2-2A PIC not aware of training and responsibilities about going over public health policies with employees. Not able to report symptoms or diseases to the person in charge. Repeat Violation. 2-2D Violation of Code: [511-6-1.07(3)(b)] Using customer napkins for drying provisions at hand washing sink. All hand washing sinks should have their own hand driving provisions to prevent possible contamination from dripping. Repeat Violation. 11D Violation of Code: [511-6-1.05(2)(k)] No thermometer present on food facility. All food facilities should have thermometer on site to record minimum internal cooking temperatures to prevent serving undercooked foods. Repeat Violation. 12D Violation of Code: [511-6-1.04(4)(g)] Half cut head of cabbage in cooler. Owner says extra is added to cabbage slaw. Restaurant facility doesn't have vegetable sink. Can only serve prewashed and prepared vegetables. Kitchen very small. Addition of vegetable sink may not be able to accomplish. New Violation. 13A Violation of Code: [511-6-1.02(1)(d)] Didn't have current inspection report posted. Current inspection reports should be posted within public view(at order window.) New Violation. 15B Violation of Code: [511-6-1.05(3)(h),(i)] Test strips not found on food facility to test concentration of sanitizer within 3 compartment sink. All Facilities should have test strips available to test sanitizer concentration. Repeat Violation. 15C Violation of Code: [511-6-1.05(7)(d)] Hood vents and inside hood need cleaning from accumulation of grease. Need frequent cleaning to prevent dripping grease on food and food contact surfaces. New Violation. 17D Violation of Code: [511-6-1.07(4)(b)] Personal frozen food items found within cooler not listed on restaurant menu. (Hamburger(ground beef - frozen patties used at restaurant), sausages). Personal food items may not be stored within same area as personal restaurant food items. If these items are stored at restaurant then they must be stored away from restaurant stock in separate area. New Violation. 18 Violation of Code: [511-6-1.07(2)(m)] Hole is still beneath 3 compartment sink cabinet leading to outside. Openings should be closed to prevent possible entry of pest. Repeat Violation. 18 Violation of Code: [511-6-1.07(5)(k)] Still observing roaches inside facility. Needs to provide proof of treatment for insects by licensed pest control operator within 10 days. Repeat Violation. 18 Violation of Code: [511-6-1.07(2)(m)] Door of shed and door to trailer food service are being left open allowing for the entry of flies into food preparation and food storage areas. Doors should be tight fitting and self closing. Door to the dry storage can be left closed if not being used. Repeat Violation.
