Violation of Code: [511-6-1.03(5)(j)1&2] Open Styrofoam cup of coffee on shelf below steam table in main kitchen beside prep top cooler. Drinks are allowed in kitchen in disposable cup with a secure lid and straw and stored to prevent any possible contamination of food/food contact surfaces. COS: Employee discarded open drink. Corrected On-Site. New Violation. 5-2 Violation of Code: [511-6-1-.04(7)(e)] Menus that are given to customers did not have a consumer advisory for animal fooods customers may order cooked to their liking - hamburgers and salmon. Menu must have salmon and hamburgers marked with and asterisk and a reminder statement (Consumer advisory) must coincide with those menu item (also marked with and asterisk)the customer menus. COS: Person in Charge stated the copy on computer has advisory and is going to reprint menus. New Violation. 6-1A Violation of Code: [511-6-1.04(6)(f)] 2-door prep cooler was not able to hold time and temperature control for safety food items at required 41F or below. Items were prepped the day prior and placed into cooler following morning and were out of temperature. See temperature observations. Condensate leaking into bottom of cooler. COS: Employee discarded all Time and temperature control for safety foods that were not holding temperature. New cooler has been on order. Calling maintenance to service if possible. If needed for service , store all time and temperature control safety foods on ice until unit repaired and can hold food at 41F or below or new unit arrives to hold food at 41F or below. Corrected On-Site. Repeat Violation
