Violation of Code: [511-6-1.05(6)(n)] No sanitizer registering on chlorine test strips at in-use warewashing machine. PIC ran multiple cycles, no sanitizer visually delivered through dispenser tube into machine. PIC placing maintenance call to have machine serviced and to use 3 compartment sink to manually WRS utensils until machine properly functioning. Repeat Violation. 4-2B Violation of Code: [511-6-1.05(7)(a)1] Food residue observed on food contact portion of stored power operated mixing wand and can table mounted can opener blade. Accumulated residue observed along interior surfaces of bulk sauce containers in walk-in cooler. Accumulated food debris observed on food contact surfaces of stored pans, baking sheets, and containers. Clean food contact surfaces to sight and touch before sanitizing for storage/reuse. PIC- waiting on dishwashing employee to arrive for shift to clean discussed items before use. New Violation. 4-2B Violation of Code: [511-6-1.05(7)(b)] Accumulated reside along interior food contact surface of ice chute at drink dispenser. Clean surface frequently and thoroughly enough to preclude accumulations. PIC- to obtain reaching brush to give chute deep cleaning and sanitizing. New Violation. 6-1A Violation of Code: [511-6-1.04(6)(f)] Provolone, sliced turkey, and sliced roast beef 50-51F in prepline cooler. Foods must maintain 41F or less when held cold. COS- PIC Voluntarily discarded foods. Foods were not prepared this a.m. per PIC- after taking internal food temperatures throughout unit, pans in center of flip-top portion of unit is area where foods are unable to maintain 41F. PIC to place service call and have unit inspected, center pans to not be used for cold-holding until PIC can verify temperatures are 41F or less. Corrected On-Site. Repeat Violation. 6-2 Violation of Code: [511-6-1.04(6)(g)] Bulk pizza sauce and meat sauce have no date labels- sauces made 9/8/17 per PIC. Containers of pastas and cooked/portioned lasagnas and calzones in coolers have no date labels- foods prepared on previous days and dates per PIC. Foods prepared in-house and kept for more than 24hours must be date labeled to where staff can track date of preparation and ensure product is discarded within 7 days. COS- PIC able to label foods from original date(s) of preparation, to discuss labeling practices with staff. Corrected On-Site. Repeat Violation. 17A Violation of Code: [511-6-1.07(5)(h)] Accumulated debris and build-up on handles at kitchen handwashing sink. Keep fixtures clean of accumulations to reduce the potential for cross-contamination when washing hands. New Violation. 17B Violation of Code: [511-6-1.06(5)(k),(l)] Dumpster lids broken off and missing. Keep containers used for holding refuse maintained and in good repair. New Violation. Remarks Have warewashing machine service technician install new data plate or sticker that states minimum operating requirements of machine's water temperature and sanitizer concentration so staff can reference when operating- current label deteriorating and unable to adequately read. LH inspected with AR.
