Violation of Code: [511-6-1.03(2)(m)] No verifiable way to confirm that employees have been educated on need to report to the health authority certain symptoms and illnesses that are most transmissible through food. COS: Provided establishment with a copy of reporting guidelines Corrected On-Site. New Violation. 2-2E Violation of Code: [511-6-1.03(6)] PIC unable to provide or explain a procedure for cleaning up contamination events. An establishment must have a procedure in place for cleaning up vomitus and diarrheal events. COS: Provided contamination clean up instructions to establishment Corrected On-Site. New Violation. 6-2 Violation of Code: [511-6-1.04(6)(h)] Salsa para with prep date of 8/23/17 found in sandwich unit. Actively used for 3 days after required disposition date. Ready to eat TCS foods must be discarded within 7 days of preparation. Day one is counted as the date of prep. COS: Salsa para voluntarily discarded Corrected On-Site. New Violation. 15C Violation of Code: [511-6-1.05(7)(a)2,3] Outsides of most containers found with leftover residue from date label stickers. Nonfood contact surfaces of equipment must be cleaned frequently enough to prevent the accumulation of debris. New Violation. 17B Violation of Code: [511-6-1.06(5)(o),(p)] Trash and debris littered behind dumpster inside of enclosure. Dumpster enclosures must be maintained for cleanliness to prevent odor and attraction of pests and rodents New Violation.
