Inpection Notes
Violation of Code: [511-6-1.03(2)] No designated Person In Charge in the absence of the CFSM. When CFSM is not on site, there must be a person in charge on the premises to monitor food safety. Repeat Violation.
2-2A Violation of Code: [511-6-1.03(2)(m)] No verifiable means of ensuring employees were informed of symptoms associated with diseases that can be transmitted through and of prevention of transmission of foodborne disease by employees who have a disease or medical condition that may cause foodborne disease. New Violation.
2-2B Violation of Code: [511-6-1.03(5)(j)1&2] Employee coffee drink in Styrofoam cup without lid left out on food prep surface and on table next to blender. Employee water bottle stored on prep surface in kitchen. Employees may consume beverage if in disposable cup with lid and straw and stored in a designated area for employee consumption.
COS-Drinks voluntarily discarded. Repeat Violation.
4-2B Violation of Code: [511-6-1.05(6)(n)] No sanitizer registering on chlorine test strips for container holding wiping cloths used to wipe prep surfaces. Chlorine sanitizer must be maintained at a concentration of 50-200PPM.
COS: Bleach added to bucket New Violation.
6-2 Violation of Code: [511-6-1.04(6)(g)] Deli ham slices stored in reach in cooler for several days not marked with a 7 day discard date. Ready to eat TCS foods must be date marked with a 7 day date with the date of preparation, or opening, counting as day 1.
COS: CFSM was able to determine when ham was sliced and labeled containers with a date mark Corrected On-Site. New Violation.
12A Violation of Code: [511-6-1.04(4)(q)] Bucket of sugar sitting on floor in main prep area. To prevent any possible contamination, all food storage must be at least 6" above the floor.
COS: Sugar moved to top of empty bucket Repeat Violation.
12C Violation of Code: [511-6-1.04(4)(m)] Cloths used to wipe prep tables found damp and sitting out on counters between use. Between uses, store cloths used to wipe food contact surfaces in a sanitizer solution to control he growth of bacteria.
COS: Wiping cloths moved to bucket Repeat Violation.
14A Violation of Code: [511-6-1.04(4)(k)] Container of flour storing rolling pin, sifter handle, dough cutter, and bush with handles contacting the product. Maintain handles of equipment above the top of the food during storage.
COS: Utensils removed from flour bin and taken to 3 compartment sink Corrected On-Site. New Violation.
15B Violation of Code: [511-6-1.05(6)(d),(e)] 3-compartment sink sanitizer compartment with accumulation of greasy residue. Sinks shall be cleaned at a frequency throughout the day to prevent recontamination of equipment and utensils and/or at least every 24 hours. Repeat Violation.
