Violation of Code: [511-6-1-.04(7)(e)] No consumer advisory on to go menu for hamburgers offered cooked to customer requests (undercooked). Consumer advisory on wall in dining room does not mention items that can be cooked to customers request and there is no posted menu in dining room to refer to. Discussed with CFSM adding asterisk next to hamburgers and adding consumer advisory on to go menu and as well as adding hamburger specifically on consumer advisory hanging on the wall in dining room. New Violation. 14B Violation of Code: [511-6-1.05(10)(g)2] Unwrapped eating utensils (knives, forks and spoons)for customers self-service are in containers where the food/lip contact part of the utensil can be touched by customers. Unwrapped eating utensils out for customer self-service must be stored so that only handle of the utensil is touched when customer picks-up utensil to avoid contamination of food/lip contact surface. COS: CFSM turned all the knives, forks and spoons upside down so only handle is presented. Corrected On-Site. New Violation. 16C Violation of Code: [511-6-1.06(4)(c)] New vegetable sink installed during down time of facility is not gapped to prevent backflow. To prevent contamination of sinks that would contain food/food contact surfaces an air gap must be installed to prevent a complete connection between sink drain and sewage system that would allow for backflow. New Violation.
