Violation of Code: [511-6-1.03(2)(m)] No verifiable way to confirm employees have been trained in their responsibility to report information about their health and activities as they relate to diseases transmissible through food to the person in charge. COS: HA supplied reporting responsibility forms (k-30) to the establishment Corrected On-Site. New Violation. 4-2B Violation of Code: [511-6-1.05(7)(b)] Can opener with a buildup of dried food debris. Food contact surfaces of equipment must be cleaned at a frequency necessary to prevent the build up of dirt and debris. COS: Can opener washed, rinsed, and sanitized. Corrected On-Site. New Violation. 4-2B Violation of Code: [511-6-1.05(7)(a)1] Tray used to hold clean utensils contained dirt and crumbs on the bottom. Equipment food contact surfaces and utensils must be clean to sight and touch COS: Tray washed, rinsed, and sanitized Corrected On-Site. New Violation. 6-1A Violation of Code: [511-6-1.04(6)(f)] Yogurt in display case cooler recorded at 46-51 degrees F. Half and half, and cream stored in ice bath measured at 61 degrees and 55 degrees F, respectively. Sandwiches prepared yesterday held in rear makeline recorded at 53 degrees F. Time and temperature control for safety foods must be cold held at 41F or below. COS: Foods voluntarily discarded. Maintenance called to check on coolers. Corrected On-Site.
