Violation of Code: [511-6-1.05(7)(d)] Accumulated food debris hanging from wire racks in walk-in cooler. Labels on racks discolored and holding debris and moisture. Surfaces of paper towel dispensers, soap dispensers, handsink basins, A/C unit by back handsink, and warewashing area floor fan have accumulated build-up or dust. Handles to bulk food containers and cooking/cold holding equipment have accumulated debris. Clean equipment surfaces frequently and thoroughly enough to keep rid of accumulations. Repeat Violation. 16B Violation of Code: [511-6-1.06(3)(b),(c)] Riser lids for grease trap and septic tanks not secured in place and easily come off when touched or bumped. Routinely check (recommend incorporating into the daily check log) the septic system to ensure lids are secure and unintentional falling or entrapment from unsuspecting persons cannot occur. COS- Screws for septic lids were not aligned with anchors, CFSM realigned lids and tightened screws to secure lids in place. Corrected On-Site. Repeat Violation.
