Violation of Code: [511-6-1.03(1)(a)-(c)] PIC failed to demonstrate satisfactory control of risk factors that can be associated with foodborne illness by allowing the food service establishment to continue to operate without keeping hot water heater on fro hot water to wash dishes, wash hands, describing the relationship between foodborne disease and the personal hygiene of employees, allowing employee that was observed at start of inspection to brush teeth at 3-compartment sink, identifying toxic materials in spray bottles in the facility, and maintaining the facility in a clean condition and in good repair. It is the duty of the PIC to monitor food safety at the food service establishment. New Violation. 2-2A Violation of Code: [511-6-1.03(2)(m)] PIC has not informed employees in any manner - required to be in a verifiable manner - of their responsibility to report to the person in charge , information about their health as it relates to diseases transmissible through food. COS: Discussed this responsibility with the PIC and left form for PIC to go over with employees and employees can sign. Corrected On-Site. New Violation. 2-2D Violation of Code: [511-6-1.06(2)(c)] No hot water at hand sink at start of inspection. Informed by PIC that hot water heater needs repair (leaking) and hot water has been turned off. Plumber contacted but Estimated time of arrival is to be between 1:00PM and 7:00PM. COS: Owner turned hot water heater back on in lieu of closing until water heater repaired. Corrected On-Site. New Violation. 2-2D Violation of Code: [511-6-1.06(2)(o)] At start of inspection, used utensil laying in hand sink - scoop, spoon. A hand sink shall only be used for the washing of hands. COS: Utensils moved out of the hand sink. Corrected On-Site. New Violation. 2-2E Violation of Code: [511-6-1.03(6)] No procedures in place for employees to follow in responding to a vomit or diarrhea event that may occur in the establishment. Must have procedures in place to clean and sanitize properly to prevent the spread of contamination to employees and customers should an event occur. COS: PIC given guidelines for the clean-up of vomit/fecal events and instructed to share procedures with employees and keep procedures available for reference. Corrected On-Site. New Violation. Violation of Code: [511-6-1.05(7)(b)] Potato slicer with dried food debris from last use in push plate. Inside ice machine - on top around and in chute with large build-up of mold. Tea urn spout with build-up of mold in spout/on nipple. Inside ice machine needs cleaning as frequently as necessary to not have accumulation of mold, tea urn and nozzles need to be cleaned every 24 hours and potato slicer needs to be cleaned after use (sanitize all after cleaning). Corrected On-Site. Repeat Violation. 8-2B Violation of Code: [511-6-1.07(6)(b)] Many unlabeled spray bottles of chemicals in kitchen. Spray bottles of chemicals must be labeled with at least the common name of the chemical inside to ensure proper chemicals are used as required and in case of accidental poisoning. COS: PIC labeled bottles. Corrected On-Site. New Violation. 12A Violation of Code: [511-6-1.04(4)(q)] Could not enter into Walk-on freezer for all the food items sitting on the floor. All food storage must be 6" off the floor to protect food from any possible contamination and to allow for easy cleaning of floors. New Violation. 12B Violation of Code: [511-6-1-.03(5)(j)4] Employee observed brushing teeth and spitting into rinse compartment of 3-compartment sink. This activity must be performed in the restroom if teeth must be brushed at work. CODS: PIC discussed with employee brushing teeth in restroom and rinse compartment of 3-compartment sink was cleaned and sanitized. Corrected On-Site. New Violation. 14B Violation of Code: [511-6-1.05(10)(g)2] Plates/bowls at food bar not under sneezeshield or otherwise protected from contamination. Need to invert plates/bowls or somehow fit them under the sneezeshield. New Violation. 15A Violation of Code: [511-6-1.05(6)(a)] Condensate leak in 2-door sandwich unit. Pooled water in bottom of unit and hole drilled in bottom to allow water to accumulate in a pan underneath the unit. This water is considered nonpotable and should not be allowed to accumulate in this unit. Heavy ice build-up in walk-in freezer. This unit needs immediate defrosting. New Violation. 15C Violation of Code: [511-6-1.05(7)(d)] Handles of coolers, shelving (above sinks and below prep tables) bulk food containers (lids and outsides of containers), gaskets of cooler doors all with build-up of food/grease residue. Containers at food bar to hold clean plates/bowls with build-up of grease/dust in the bottoms. To prevent any contamination of clean and sanitized equipment and hands (handles) and to prevent possible insect/rodent attraction, all these areas must be cleaned on a more frequent basis. New Violation. 17C Violation of Code: [511-6-1.07(5)(a),(b)] Floors throughout kitchen - especially at floor/wall junctures need a deep cleaning - accumulation of grease and dirt can (and has been) and attraction for insects/rodents. Walls with build-up of grease/food throughout kitchen also need thorough cleaning. New Violation. 18 Violation of Code: [511-6-1.07(5)(k)] Many live roaches in the establishment - treatment occurred today. Need to prevent roaches driven from harborage areas from contacting food/food contact surfaces as much as possible. Many holes in the walls in the kitchen and buffet area that have allowed for harborage areas for insects. Holes need to be patched to prevent these areas. New Violation.
