Violation of Code: [511-6-1.03(2)(a)-(l)(n)(o)] More active managerial control needed in areas of: proper cleaning and sanitizing of equipment and utensils, procedures when using time as a public health control for uncooked dough and pizza, and physical facility cleaning and maintenance. It is the responsibility of the PIC/CFSM to maintain a written contract with key drop delivery providers stating their responsibility of control measures in place that address conditional employee health, temperature control for TCS foods, and secured access for delivered foods. Key drop information packet furnished to CFSM. New Violation. 4-2A Violation of Code: [511-6-1.04(4)(c)1(iv)] Bulk container of flour stored on prep table counter with no cover or lid, keep foods covered when stored between uses. COS- lid placed on container. Corrected On-Site. Repeat Violation. 4-2B Violation of Code: [511-6-1.05(7)(a)1] Accumulated food debris and residues on food contact surfaces of stored utensils (pizza cutters, tongs, ladle, pans, metal tubs, and plastic containers). Clean utensils to sight and touch before sanitizing for storage/reuse. Repeat Violation. 15B Violation of Code: [511-6-1.05(3)(b),(c)] Dirty utensils stacked on drainboard and basins of 3 compartment sink with no area or space available to place utensils to set up sink compartments correctly wash, rinse, and sanitize. Staging areas (i.e. drainboards, utensil racks, or tables) must be large enough to separately accommodate all soiled and cleaned items that may accumulate during hours of operation for necessary utensil holding before cleaning and after sanitizing. Based on current size of utensil staging area, utensils must be cleaned more frequently throughout the day. New Violation. 15B Violation of Code: [511-6-1.05(6)(d),(e)] Accumulated food debris and greasy film on surfaces of 3 compartment sink basins and clean side drainboard for air drying. The surfaces of equipment used for manual warewashing must be cleaned before use and throughout the day at a frequency necessary to prevent recontamination of utensils being cleaned. New Violation. 15C Violation of Code: [511-6-1.05(7)(d)] Accumulated debris and residues on non-food contact surfaces of Hobart dough mixer, table top dough stretcher, speed racks, and rack shelving at warewashing area. Clean equipment surfaces frequently and thoroughly enough to keep rid of accumulations. New Violation.
