Violation of Code: [511-6-1.03(3)(c)] No documentation could be furnished to demonstrate a CFSM on staff. Previous CFSM certification expired 1/19/17. Need proof of exam registration within 30 days. New Violation. 2-2D No hot water available at handwashing sinks in men's or women's restroom. Handwashing sinks must provide water at a temperature of at least 100F. Have 14 days to correct. Repeat Violation. 15C Violation of Code: [511-6-1.05(7)(d)] Top and side of under the counter grease interceptor at 3 compartment sink has accumulated grease like build-up. Clean exterior surfaces of interceptor frequently and thoroughly enough to keep rid of accumulations. New Violation. 17A Hardware on door handle to men's restroom stripped to where upon leaving the restroom, the door will look behind patron if lock button engaged and cannot be re-opened without managerial assistance. Restrooms must be kept accessible during all hours of operations- need locking hardware replaced. Have 14 days to correct. Repeat Violation. 17A Violation of Code: [511-6-1.07(5)(h)] Sinks and toilets in both restrooms dirty, plumbing fixtures must be kept clean and maintained. Repeat Violation. 17A Violation of Code: [511-6-1.06(2)(h)] Women's restroom toilet wobbles and shifts when weight is applied to seat, needs repaired to where securely seated to floor and meets plumbing code requirements. Have 14 days to correct. New Violation. 17C Violation of Code: [511-6-1.07(1)(a)] 2 areas of FRP wall in women's restroom has been patched with a spackle-type material that cannot be easily cleaned and holds moisture when splashed. Restroom wall materials and coverings must be designed, constructed, and installed so that they are non-absorbent, smooth, and easily cleanable. Have 14 days to correct. New Violation. 17D Violation of Code: [511-6-1.07(3)(g)] Obnoxious urine odor in men's restroom. Check exhaust ventilation to ensure device is properly working. New Violation.
