Violation of Code: [511-6-1.03(5)(j)1&2] Observed employee drinking from coffee mug at food prep station while slicing onions. If working in a food prep area, employees can only drink from a single service beverage cup with a lid and straw

2-2D Violation of Code: [511-6-1.06(2)(o)] Stack of empty buckets blocking access to handsink beside 3 compartment sink. A handwashing sink must be maintained so that it accessible at all times.

4-2B Violation of Code: [511-6-1.05(6)(n)] Sanitizer bleach bucket very cloudy and not registering any chlorine on test strips. Chlorine sanitizer must be maintained between 50-100 PPM at room temperature and should be changed when cloudy.

6-1A Violation of Code: [511-6-1.04(6)(f)] Cooked shrimp, cream cheese, and egg rolls in walk in cooler found at 46-47F. Time and temperature control for safety foods must be cold held at temperatures of 41F or below. Have walk in cooler checked by service provider.
