Violation of Code: [511-6-1.05(7)(a)1] Metal pans stacked above 3 compartment sink built up with old food debris, grease, and sticker residue. Disassembled tea nozzles found to contain mold build up. Food contact surfaces of equipment must be clean to sight and touch. Correct by 5/31/17 New Violation. 4-2B Violation of Code: [511-6-1.05(6)(n)] Quat sanitizer bucket for makeline cooler testing at less than 100PPM. Change bucket more frequently to hold concentration between 200-400PPM. COS: Contents of bucket discarded and refilled with solution at 200PPM Corrected On-Site. New Violation. 6-1A Violation of Code: [511-6-1.04(6)(f)] Temperature of mashed potatoes, pasta, shredded cheese, parmesan cream sauce, tomatoes, and lettuce stored in makeline cooler all above 41F (see log). Time and temperature control for safety foods must be cold held at 41F or below. COS: An ice bath was used on all the foods, which had only been in the unit for 3 hour, to cool to 41F. Maintenance called to service. Corrected On-Site. New Violation. 15A Violation of Code: [511-6-1.05(6)(a)] Push block part of vegetable dicer with edge of block sheared off and ragged - this piece needs to be replaces since pieces can shear off and end up in the food that is being prepared COS: Manager discarded damaged block Corrected On-Site. New Violation. 15C Violation of Code: [511-6-1.05(7)(d)] Shelving throughout kitchen, handles of equipment, and insides of coolers with build-up of grease and food debris. Non-food contact surfaces shall be cleaned at a frequency to prevent the build-up of accumulations. Repeat Violation.
