Violation of Code: [511-6-1.03(3)(c)] No CFSM certificate posted at the food service establishment. The original CFSM certificate shall be posted in public view in the food service establishment. New Violation. 2-2A Violation of Code: [511-6-1.03(2)(m)] Shift leader/PIC (only employee on duty) could not recall 5 symptoms of illnesses that could relate to disease transmissible through food and could not name any of the 6 foodborne illnesses that need to be reported to the CFSM and PIC if diagnosed with these illnesses. COS: Left Employee Health Red Book for shift leader to read and Corrected On-Site. New Violation. 2-2E Violation of Code: [511-6-1.03(6)] No procedure to handle clean-up of vomit or diarrheal incidences could be found. Procedure must be where all employees know where to find it so proper clean-up can be done to prevent spread of illness to customers and other employees. COS: Procedure from State website given to shift-leader. Corrected On-Site. New Violation. 4-2B Violation of Code: [511-6-1.05(7)(a)1] Cleaned and sanitized equipment and utensils stored with food residue (blades of knives on knife rack) and greasy - lexan containers, metal strainer pans greasy and with food residue. After cleaning and sanitizing kitchenware/utensil check for cleanliness before storage. COS: All kitchenware/utensils found greasy and with food debris were piled up at the 3-compartment sink for better cleaning and to be sanitized again. Corrected On-Site. New Violation. 4-2B Violation of Code: [511-6-1.05(7)(b)] Inside of ice machine with build-up of mold around chute, on back of baffle and inside top of bin. To prevent build-up of mold, the ice machine needs to be cleaned and sanitized more frequently - at least once a month. Dining tables being cleaned and sanitized with spray sanitizer but, a cloth is being used instead of a disposable towel as required. If cloth used, must have 2 buckets of sanitizer and 2 separate cloths using on bucket/cloth for cleaning and a the second bucket/cloth for sanitizing. COS: Paper towel to be used to wipe tables after spraying with sanitizer. Corrected On-Site. New Violation. 11A Violation of Code: [511-6-1.04(6)(e)] Raw chicken wings being held in bus tub on top of ice in lower bus tub where ice only contacting bottom of top buss tub where most wings are not on bottom of bus tub. Top bus tub should be sunk completely into ice to maintain required cold holding of all chicken and not just the ones on the bottom. COS: Only a few wings left in bus tub after 1 hour - those wings cooked. Not storing so much out in bus tub since cooler right next to fryer until storage can be figured out. Corrected On-Site. New Violation. Violation of Code: [511-6-1.02(1)(d)] No inspection report posted. Must post most current inspection report in public view. COS: This inspection will be posted. Corrected On-Site. New Violation. 13A Violation of Code: [511-6-1.02(1)(f)(1)] Food Service permit not displayed. Records indicate Food Service Permit was mailed from our office. Try to locate or new Food Service Permit can be printed. New Violation. 15B Violation of Code: [511-6-1.05(3)(h),(i)] No test strips for type sanitizer being used - Quat sanitizer being used in cloth storage buckets, 3-compartment sink and to spray dining tables and wipe with paper towel. Must have test strips to monitor strength of sanitizer in use. New Violation. 17B Violation of Code: [511-6-1.06(5)(o),(p)] Accumulation of grease around grease container and on grease container in the dumpster area. Keep this area clean to prevent accumulation that can be attraction for insects/rodents. New Violation. Remarks Discussed getting someone at the food service establishment to be Certified Food Safety Manager (CFSM) since currently District Manager listed as CFSM but, he is not at the location often enough to provide training and monitor employees.
